SALARY RANGE: $ 23,589- $ 29,603 - $ 37,744

LOCATION: Rutherford County, NC

DESCRIPTION OF WORK:

  • Nurse-Family Partnership (NFP) is an evidence-based nurse home visiting program that improves health, well-being, and self-sufficiency of first-time families affected by economic inequality, first-time parents, and their children. The Nurse-Family Partnership National Service Office, located in Denver, CO is a non-profit organization that provides service to communities in implementing and sustaining this program.
  • This position will support services in a variety of clerical duties for the Nurse Family Partnership (NFP) Supervisor and Nurse Home Visitors (NHVs).
  • The position also includes considerable knowledge of NFP to provide community outreach for building and maintaining relationships with referral partners.
  • Meets in-person with clients referred to NFP and communicates effectively to discuss the NFP program and enrollment process.
  • Scans and inputs all NFP referral forms, Home Visit forms, and program documents in a timely and accurate manner into Disease Management Coordination Network (DMCN).
  • Contacts all NFP referrals by phone and documents in DMCN per policy. Assigns referrals to NHVs, upon direction of supervisor.
  • Assist with client outreach and reminder calls for home visits as needed.
  • Assists NFP nurses with telehealth visits, home visits, and texts in Spanish as needed.
  • Serves as a liaison and point of contact for NFP of the Foothills, community referral sources, and health care agencies.
  • Performs program support tasks such as organizing forms, files, orders and educational handouts.
  • Bi-lingual experience preferred.

MINIMUM EDUCATION AND EXPERIENCE:

Graduation from High School and demonstrated possession of knowledge, skills and abilities gained through at least one year of office assistant/secretarial experience; or an equivalent combination of training and experience.

KNOWLEDGE, SKILLS AND ABILTIES:

General knowledge of office or work unit procedures, methods and practices. Ability to screen communications based on predetermined guidelines to independently respond or route inquiries. Ability to maintain patient confidentiality as well as the ability to deal professionally with the public.
